I also acknowledge my access to my online personal health record, including the ability to communicate with my healthcare team regarding my health information through Athlete Health & Performance’s application. I understand that in certain states, medical professionals are legally prohibited from electronically releasing specific test results to me, and therefore, I may not have access to all of my health information online within my personal health record. I acknowledge that Athlete Health & Performance reserves the right to restrict or terminate my use of online patient services if I fail to comply with these terms and conditions or at the discretion of Athlete Health & Performance.

Identity Verification I understand that my enrollment is subject to Athlete Health & Performance verifying my identity as deemed appropriate. This may include in-person verification or other methods.

Referrals Nothing on this website (including links to other websites) should be construed as promoting, referring, recommending, endorsing, guaranteeing, or warranting any particular healthcare service, product, entity, or provider.
